THE GAME TIME PERFORMANCE APPROACH
Here's how our 6-phase approach to hamstring rehab is UNMATCHED IN THE INDUSTRY
1. PROTECTION PHASE
Aims of stage:
-
Reduce swelling and symptoms.
-
Ensure surgical incision hygiene is maintained.
-
Allow the knee to settle, allowing for low load exercises to be introduced without symptom regression.
Rehab during this stage:
-
The hospital will provide basic range of motion and activation exercises. We recommend patients stick to those over the first 1-2 weeks.

If patients have been prescribed a brace/splint by the surgeon, we recommend adhering to their advice, as this will help ensure optimal surgical outcomes and faster rehab timeframes.
2. PRIMARY LOADING
Aims of stage:
-
No swelling or symptoms with everyday life activities.
-
Restore full range at the knee and normal walking patterns.
-
Regain a base level of muscular endurance/capacity and single leg balance.
Rehab during this stage:
-
Physio 1 - 2 x a week.
-
Manual therapy to help reduce symptoms and improve range.
-
Physiotherapist-led exercise therapy sessions, with progressive implementation of Compex technology and blood flow restriction training.

Tailored home programs via our mobile app.
3. STRENGTH
Aims of stage:
-
Regain strength of all major muscle groups within 10% of non-injured side
-
Progress through low-level plyometric program to prepare for running loads.
Rehab during this stage:
-
Fortnightly physiotherapy, 2-3 gym rehab sessions per week (in-person or following an online program).

Physiotherapy consultations will involve retesting of key metrics, manual therapy where relevant and/or exercise therapy sessions to evaluate knee function and program accordingly.

Now that the knee is “quiet” and a full range is available, the most important part of the rehab program is the rehab/exercise component.
4. POWER
Aims of stage:
-
Regain relative symmetry in rate of force development (speed of contraction) during lower limb strength testing.
-
Achieve optimal lower limb strength based off bodyweight normative values.
-
Reintegrate straight line running and sprinting.
-
Prepare for change of direction and agility work in the next stage of rehab.
Rehab during this stage:
-
Fortnightly physiotherapy, 3-4 gym rehab & field sessions per week (in-person or following an online program).
5. RETURN TO TRAINING
Aims of stage:
-
Maximise physical preparation for a successful return to their sport/activity.
-
Reintegrate change of direction drills, progressing from planned to unplanned variations.
-
Return to sports training, progressing over time, achieving a minimum of 2-4 weeks of non-restricted training prior to a sporting return.
Rehab during this stage:
-
4-weekly physiotherapy, 3-4 gym rehab & field sessions per week (in-person or following an online program).
-
Athletes will return to sports trainings with clear instructions on what they can/cannot do, with all parties (coach, player, physio) on the same page.
6. RETURN TO PERFORMANCE
One of the biggest mistakes we see ACL patients make is that once they are back playing sport, their physical preparation and injury reduction work drops significantly.
Unfortunately, up to a third of patients who return to sport will suffer a secondary injury, with nearly 50% within the first two months of returning to sport. Females have been shown to be over 5 times more likely to injure the ACL on the other side compared with males (26% vs 5%).
With rather alarming statistics for those that do return to sport we encourage all athletes to continue on with us as a Game Time Athlete.
